Waiver

In Consideration of your accepting this entry, I, intending to be legally bound, hereby, for myself, my heirs, executors and administrators, waive and release any and all rights and claims for damages I may have against the C.U.R.B. Organization, S.H.I.F.T. Foundation, the City of Long Beach, their representatives, successors, and assigns for any and all injuries suffered by me in this event.  I attest and verify that I am physically fit and have sufficiently trained for the completion of this ride and my physical condition has been verified by a licensed medical doctor.

Release

I hereby grant permission to the SHIFT Foundation, Inc. to use photographs and/or video taken on August 12, 2023 in publications, news releases, online, and in other communications related to the mission of the C.U.R.B. Bike Ride.
